[Bug] Context not retained across repeated operations, specifications not followed

Bug Description
absolute garbage today even by garbage day standards. like all I do is yell at it, it admitting it did an ass job and then doing the same mistake again half an hour later and then again half an hour later and again... its abysmal. it's as if it can't see context nor does it even follow specs...

Opus or Sonnet on any level, any Claude Code version.
