[Bug] Claude ignores user directives to wait for approval before making production changes

Question

**Bug Description**
Claude is extremely eager to make changes without approval.  I told him many times to wait for confirmation before making changes to prod.  It claims to have understood, made a memory, etc.  And it does it again.  This is really bad.
