[Bug Report: Degraded model accuracy and increased error rates

Bug Description
Claude has been acting super stupid lately, is far from the accuracy it had and I don't trust in him anymore. Our company is fully invested in working with Claude but we are facing too many errors lately. Please review and fix, this is unbearable and if there's anything abundant nowadays is AI offerings with a good track record.
