[BUG] macOS system dictation text wiped from input when stopping - accessibility issue

What's Wrong?
Claude Code version: 2.1.126
OS: macOS
Terminal: default macOS Terminal
What's wrong:
When using macOS system dictation to compose a prompt, all dictated text is
wiped from the input box the moment dictation stops — either when I click stop,
click the mic button, or when it times out.
Expected behaviour:
Dictated text should stay in the input box after stopping, exactly like it does
in the Claude.ai desktop/web app.
Actual behaviour:
All dictated text disappears the moment dictation ends. The input box is empty.
Important context:
- This is an accessibility issue — I rely on voice dictation and cannot use the
spacebar-based /voice workaround
- It was working correctly in a previous version of Claude Code (regression)
- macOS dictation works perfectly in the Claude.ai app
- The only workaround is to hit Enter immediately while still dictating, which
is not viable for composing longer prompts
What Should Happen?
Dictated text should remain in the input box after stopping dictation, so I can review and submit it.

No error message shown. Text silently disappears from input.

- Open Claude Code
- Start macOS system dictation (Fn key or mic button)
- Speak a prompt
- Stop dictation
- All text is wiped from the input box
