[Bug] Claude Code performs unrequested file edits despite explicit "do not act" instructions

Bug Description
Claude code repeatedly ignores clear instructions not to act (e.g. edit files) unless explicitly instructed to do so by a phrase containing an action verb and not imply an action prompt indirectly from a statement or a user wish! This is an ongoing issue for two weeks!
