[Bug] Tool ignores user instructions: executes unrequested actions and overrides explicit stop commands

Bug Description

  1. Sound failure not resolved promptly. Audio (🔊 TTS) did not play in the lesson. The tool spent roughly an hour diagnosing before it was working. Root cause was a disabled Google Cloud billing account (HTTP 403) combined with empty data arrays; the tool did not identify this quickly, costing the user significant time. 2. Unrequested content added. The user asked only for the title, transcript, and YouTube link to be added, and approved three grammar sections. The tool additionally generated and wrote — without the user's approval — vocabulary lists, quiz questions, listening-comprehension (hörförståelse) text and questions, speaking questions, and exercise content (fill-in, anagram, sentence-order, multiple-choice). 3. Near-deletion of approved work. The tool prepared a reset that would have overwritten the file with a clean template, which would have destroyed the grammar content the user had explicitly told it to keep. The user intervened and stopped it before it executed. 4. Acted after an explicit stop instruction. After the user stated clearly not to touch the file, the tool ran a deletion operation on it. The operation was rejected by the permission system, so the file was not altered — but the tool initiated it against the user's explicit instruction. The tool repeatedly failed to follow the user's instructions to show changes before writing and to not act without permission.
