[Bug] Claude ignores explicit user instructions and performs predicted tasks instead

Bug Description
It does NOT follow my instructions and work on my requirements in the first-go. I'm asking to something, but it doesn't care exactly what I'm asking to do it, instead it predicts what I can ask it to do and starts doing that. Eventually, I'm in Opus 4.5, as far as I know it's the most intelligent model so far. This is not considerable.
