[Bug] Ctrl+Alt+K keyboard shortcut fails to send selected content snapshot

Bug Description
Ctrl+Alt+K for inserting IDE lines reference not longer sending snapshot of the selected content. Previous, When click Ctrl + Alt + K when selecting multiple lines, the snapshot of those selected content will be send to claude code (not visible to user tho). So user can edit the file, claude code still get the content at the moment when user click Ctrl + Alt + K . Now the snapshot no longer being send. When insert lines reference and then edit the lines, claude code cannot correctly located the lines reference content because the lines have changed.
