[BUG] Alt+M keyboard shortcut not working for plan mode

Description
The Alt+M keyboard shortcut to enter plan mode is not working. User has to manually type /plan instead.
Expected Behavior
Alt+M should open plan mode (as documented in keyboard shortcuts).
Actual Behavior
Alt+M does nothing. User must use /plan command as workaround.
Environment
- Claude Code CLI on Windows (MSYS/Git Bash)
- Same session experiencing other UI bugs (#20171, #19888)
Notes
May be related to the UI state issues reported in #20171 (phantom generating state) - possible keyboard event handling problem.
