[BUG] API Error: Rate limit reached on every command including claude logout despite 3% usage

What's Wrong?

Description:
Every CLI command, including claude logout, immediately returns API Error: Rate limit reached. The CLI is completely unusable.
/usage shows:

Current session: 3% used (resets 3pm America/Toronto)
Current week: 2% used (resets May 6, 1am America/Toronto)

What Should Happen?

Requests should succeed well within plan limits.
