[Bug] Session termination not respecting configured conversation continuity settings

Bug Description
Claude session has stopped 4 times now, once with an actual fabrication of a reason, the others with a tell me to keep going, or this is a natural stopping point etc. This is new bahaviour over the last 24 hours and in direct violation of the CLAUDE.md and MEMORY.md
