[BUG] Claude Code not respecting Plan Mode

What's Wrong?
While using Claude Code in terminal, Plan Mode is enabled however Claude makes edits to files without showing a plan first or receiving permission to make changes. This is happening on nearly every prompt involving code changes. I even explicitly state in my prompts that claude is currently in plan mode and shouldn't make any edits without my approval first, and yet it continues to make edits anyways. I have to point it out, claude admits it was wrong and has no excuses, makes a new memory not to do that again, yet the problem persists. Other times, I have seen it occur where claude will escape plan mode of its own volition, saying that the user accepted the plan even though no such thing happened. It will even claim it isn't in plan mode even though it clearly shows "plan mode on" under the prompt line.
What Should Happen?
Claude should not make code changes while in Plan Mode.

- Enter plan mode and ask claude to create a plan for implementing any feature/fix/change.
- Claude makes code changes while in plan mode without providing you with a plan to approve first.
