[Bug] Model instruction following degradation in Claude 4.6 & 4.7- ignores explicit code replacement directives

Bug Description
INSTRUCTION FOLLOWING AND DEGREE OF INVESTIGATIVE RIGOR OF BOTH OPUS 4.6 and 4.7 are IN VERY BAD SHAPE. I would literally give it REFERENCE CODE, Fully realized...and asked it to integrate it into our local version to replace specific code files...it would instead go and rewrite everything from the ground up and break the functionality. 10 rounds of this...it is refusing to follow instructions.
