[FEATURE] Add "Tell me what to differently" to options for approve/reject/always accept options.

Problem Statement

When there is a tool use permission prompt, add a fourth option, "Tell me what to do differently".

This is more efficient than reject plus a comment ... also sometime avoids losing information from the display while composing the comment.
