[BUG] On "tell Claude what to do differently" in VSCode, focus changes to IDE instead of staying in Claude Code

What's Wrong?
On VSCode: when choosing option 3 (tell claude what to do differently), I hit enter and immediately start typing.
But focus changes for a second to the IDE for some reason before going back to Claude Code terminal, which means the first few chars are typed in code files.

Focus should stay in Claude Code

- Stay in accept edits off
- Ask it to do something
- Select "tell Claude what to do differently" and start typing immediately
