[BUG] Extra usage charges triggered despite weekly plan limit at 27%

What's Wrong?
Bug Description
Claude Code is consuming "Zusätzliche Nutzung" (extra usage / pay-as-you-go credits)
even though my weekly plan limits are far from exhausted.
## Evidence
- Weekly plan usage: 27% (All Models), 0% (Sonnet), 0% (Claude Design)
- Current session usage: 0%
- Extra usage spent: €42.10 / €43.00 (99%) — this should not have been touched
## Environment
- Plan: Pro (Max 5x)
- Platform: macOS (Darwin 25.4.0)
- **Claude Code version: 2.1.98 (Claude Code)
- Model used: Claude Opus 4.6 (1M context)
- Terminal: Ghostty

What Should Happen?
Extra usage credits should only be consumed after the weekly plan limit is reached,
not while it's at 27%.
