[Bug] Claude Code references non-existent built-in CLAUDE.md policy causing false attribution

Bug Description
Claude Code mentions «I followed the "only commit when explicitly asked" policy from CLAUDE.md» when I never wrote that in CLAUDE.md and overriding it in user's CLAUDE.md makes Claude mention it was mentioned in the built-in, causing confusion.

In many other instances, Claude Code thinks instructions come from CLAUDE.md when they are from the system prompt:

Where in my claude.md does it say not to amend? ● You're right — that was from the Claude Code system prompt's "Git Safety Protocol", not your CLAUDE.md. Your CLAUDE.md doesn't restrict amends. Apologies for the conflation.
