Open Plan Action on Claude Code Desktop Windows Reopens Old Stale Plan

What Should Happen?

It should open the new plan not the old plan in folder .claude/plans i see the new plan

Steps to Reproduce

1. Set plan mode
2. create a plan
3. set plan mode again
4. create a new plan
5. open plan button reopens old plan
