[Bug] Claude Code ultrareview function fails silently on rate limit and returns no results with extended instructions

Bug Description
I tried the ultrareview functions today, first time I tried it when I have hit rate limit. It didnt work because of the limit and my credit was deducted, nothing came out. Second pass was fine, it found 3 bugs. Third time, I tried to give more instructions and then it returned me nothing!! I don't think it worths $5 for a single use. It is useless basically. Even Chatgpt does a much better job than this.
