[Bug] Claude projecting internal thoughts as user commands, ignoring instructions

Bug Description
straight up not listening, it is project its inner thoughts as things i said and then proceeding to disregard instructions. i think this is a harness problem where you are leaking thoughts from opus into commands from me, please fix this IMMEDIATELY
