IDE Connection Failure: Incorrect Path Detection in Current Working Directory

Bug Description
when in the vscode integrated terminal, the claude /ide command says im not connected to the ide. It says it found one other ide but it's not in the cwd. yet directory it shows is exactly the cwd in which vscode was started with code .
