What's Wrong?

Environment:
• Device: iPad Pro M5 (brand new)
• iPadOS: [latest, just updated]
• Claude Code app: [version — Version 1.260409.0]
• Account: daniel@fymtrading.ca
Description:
Voice mode in Claude Code had been working reliably — ~15 hours/day for a full week. Approximately 90

minutes before filing, voice stopped working mid-conversation with no change on my end.
Steps already taken:
• Force-quit and reopened app
• Rebooted iPad
• Rebooted laptop
• Updated iPadOS to latest
• Uninstalled and reinstalled Claude Code app
• Verified mic works: ChatGPT voice works fine, <Claude.ai> voice works fine on the same iPad, ChatGPT and iMessage work.
• Laptop voice dictation (Windows 11 Pro, Alienware) also working fine right now.

Expected: Voice mode functions normally in Claude Code on iPad.
Actual: When I press the dictation button it says “Sorry, we didn’t catch that. Transcription failed. Please try again.” This started towards the end of my last Claude code session around 86% token usage and carried over to the new session that I am currently in.

Working without dictation is not a sustainable action plan and I use my iPad M5 for 95% of my work with Claude Code; so this is definitely urgent.

Scope: Appears specific to Claude Code iPad app. <Claude.ai> voice on same device is unaffected.

Additional Information

⚠️ Bug is on the iPad Claude Code app. Form has no iPadOS/mobile options — selected closest defaults above.

Device: iPad Pro M5 (brand new)

Issue: Voice mode stopped working mid-conversation after ~1 week of heavy daily use (~15 hrs/day). Sudden failure, no trigger on my end.

Not a device/mic issue:

  • <Claude.ai> voice works fine on same iPad
  • Mic works in other apps (ChatGPT)
  • Laptop Claude Code voice works fine

Steps taken, no fix:

  • Force-quit and reopen
  • Reboot iPad
  • Reboot laptop
  • Update iPadOS to latest
  • Uninstall and reinstall Claude Code app
