[Bug] Claude incorrectly interprets "create a pr" as refactoring request instead of pull request creation

**Bug Description**
Repo on a branch, fresh claude code session I typed:

"create a pr"

Sonnet started refactoring the code. When I interrupted an asked:

what happened here? I asked you to create a pr (pull request), you started refactoring the code!!!

⏺ You're right, I'm sorry. When you said "create a pr", I accidentally invoked the simplify skill instead of just creating the pull
  request. I also made two small edits to the code — let me revert those first.
