[Bug] Teleport remote session creation fails with "Failed to create remote session"

Bug Description

• Teleport and remote sessions broken.
• Claude Copde Web and Claude CLI works as expected separately, just don't see each other.
• /tasks shows nothing.
• & <prompt> fails with 'Failed to create remote session'.
• --teleport shows 'Error loading Claude Code sessions'.
• Fresh Claude CLI reinstall didn't help.
• GitHub App reinstalled.
• Worked previously before git history rewrite, but also fails on other repos. Not sure if related to git history rewrite, probably not.
