[BUG] Vercel and Notion MCP server re-auth persistently failing

What's Wrong?
- Vercel MCP tools return "requires re-authorization (token expired)" — reconnecting via Claude doesn't work
- Error reference: ofid_fe998bc42861554e
- Using Claude Code CLI
- Recurring issue most sessions. Silent authentication failure then unable re-auth
- Other MCP servers affected: Notion also failing repeatedly
What Should Happen?
After re-authorizing the Vercel or Notion integrations in Claude settings, the MCP server should authenticate successfully and tools should work without errors.

Steps to Reproduce
Steps to reproduce:
- Open Claude Code CLI
- See notification that 2 MCP servers need authorization (Notion, Vercel)
- Run /mcp to view which servers need auth
- Attempt re-authorization from the /mcp dialog
- Auth fails; must Esc out of /mcp dialog
