[Bug] Claude Code ignores claude.md rules and makes unrequested changes

Bug Description
It's become impossible to work with this tool. Rules, even those mentioned in the previous message, are constantly ignored.
Claude.md rules or other methods for enforcing rules don't work.
For example, there's a problem with a rule I'm constantly struggling with: don't make any changes until the user requests them.
As a result, after the user asks a question and waits for an answer, they immediately begin implementing it.
