[Bug] Claude 4.6 Opus ignores explicit user rules and instructions

Bug Description
I gt progression but Opus 4.6 is a regression. It assumes, guesses, and then when I make rules to guide it it FLAT OUT ignores them. I will NEVER take speed over accuracy. Back to 4.5 until you get your stuff right. Hell it just tried to make this bug Opus 3.5 WOW, and it fully borked the terminal it's IntelliJ Ultimate
