Model selector: Opus on Max plan should indicate included 1M context
Problem
The /model selector shows two Opus options:

On Max plans, option 3 already includes 1M context — per official docs:
"On Max, Team, and Enterprise plans, Opus is automatically upgraded to 1M context with no additional configuration."
But option 3 says nothing about context size. Users see option 4 with "1M context" and assume they need it, then see "extra usage" and either pay unnecessarily, avoid 1M thinking they can't afford it, or get confused about what they're actually getting with option 3.
Suggested fix
For Max/Team/Enterprise subscribers, option 3 should display the context size, e.g.:
3. Opus Opus 4.6 · 1M context · Most capable for complex work
Or hide/merge option 4 entirely for plans where 1M is included.
