[BUG] Using Max 200 Plan but Charged for API Usage!

Bug Description

I'm logged in from Claude Code using Max 200 Plan via OAuth. The usual regular API timeouts happened, but this time it's different. "Claude Console" was also unavailable. When the screen to log into Claude Console was available, OAuth login stalled. Trying to go through the chat service to get help won't work because their login fails, so no support there either. FRUSTRATING!

Steps to Reproduce

1. Use Claude Code
2. After a while, the regular API errors occur
3. You get a notice saying they charged you for API usage when you are on the Max plan

Expected Behavior

Not charge me for what I didn't use?

Actual Behavior

Charged me for $5? HUH?
