[Bug] Agent hallucinates user messages and writes incorrect memories after idle wait period

Bug Description
i told team-lead to wait, and after a few hours it completely hallucinated a message from me, and went on to write factually incorrect memories.
Environment Info
- Platform: darwin
- Terminal: tmux
- Version: 2.1.86

In Claude Code, after a long idle time, the team lead wrote a message starting "Human: ok so now I need to save memroies and also to document this so I can continue tomorrow," --- i.e. hallucinating that I had written it a message, when I hadn't.
