Regression: Opus 1M context now shows 'Billed as extra usage' on Max plan — was correctly showing 'included' until March 27

Bug description

This is a regression, not a new bug.

Until March 27, 2026, the /model selector in Claude Code correctly showed that Opus 4.6 (1M context) was included in the Max plan — no extra usage. Since March 27-28, it now shows:

4. Opus (1M context)    Opus 4.6 with 1M context · Billed as extra usage · $5/$25 per Mtok

This contradicts Anthropic's own official communication.

Proof: Official Anthropic email (March 20, 2026)

On March 20, 2026, the Claude Team sent an email to Max subscribers (from no-reply@email.claude.com, subject: "New: Claude Code channels, scheduled tasks, and more") that explicitly states:

"1M context is now included in Max, Team, and Enterprise plans — no extra usage required. Select it with /model to get longer sessions, fewer compactions, and more room for context that matters."

The problem

Changing billing terms without notifying customers is a serious issue. Users subscribed to Max plans (up to $200/mo) based on communicated features. Silently reverting the 1M context inclusion — or even just displaying incorrect billing info — erodes trust.

If the pricing actually changed: customers should have been notified before the change, not discover it in the model selector.

If the pricing didn't change and this is a display bug: it needs to be fixed urgently, as it's causing users to avoid 1M context thinking they'll be charged extra.

Options 2 and 4 should NOT show "Billed as extra usage" for Max/Team/Enterprise subscribers.
