[BUG] Rate limit reached immediately on new Max subscription despite 1% usage

What's Wrong?

• Upgraded from Pro to Max on 26 March 2026
• Session usage: 1%, Weekly: 5%
• Login method correctly shows: Claude Max Account
• Error persists after logout/login, deleting ~/.claude.json, and npm update
• Version: 2.1.85, macOS
• Getting "API Error: Rate limit reached" on every single request
• Web interface (claude.ai) works fine — issue is only in Claude Code
