Context Loss and Repeated Type Errors in Claude Code Generation

Bug Description
Claude loses the context regularly and thoroughly. It becomes quite inefficient and unusable after the first 5-6 messages. It cannot comprehend what the whole codebase is doing. it keeps on making the same mistakes of going around in circles and duplicating code. The biggest issue is with types. It creates new interfaces for everything and has to be stopped repeatedly and told not to do that. It types 'any', without fail and 100% accuracy, even when repeatedly told not to do that. Even with claude.md file setup properly. It has an utterly disgusting rate of following instructions. I believe sonnet has become unusable. Its performance is extremely degraded.
