[Bug] Claude Code model regression: degraded code quality and reliability

Bug Description
/feedback whatever updates were done, claude is now dumber to the point I have to swap to OpenAI
or Gemini cause this is detrimental to my code base and I no longer trust it on high priority
projects. A few notable changes: it no longer goes into planning mode on its own (or recognizes when it is needed), it makes significant structural and baseline code mistakes to the point I need multiples agents running to double and triple check whereas before I didn't need to do this, the intelligence of the system is downgraded. Overall it feels like I went from working with a Senior or Staff Software Engineer to a Junior Software Engineer or even an intern.
