VS Code: Disable auto-scroll when user is reading earlier messages (add 'jump to latest' button)

When reading output from earlier in the conversation, any new output or tool approval popup auto-scrolls to the bottom, losing my place and breaking my train of thought.

Expected behavior: If I've scrolled up to read earlier messages, the view should stay where I am. Show an unobtrusive "Jump to latest" indicator (similar to Slack's "new messages" pill) so I can easily scroll down when I'm ready.

Current behavior: Every new message or tool approval request forces the view to the bottom of the chat.
