VS Code: Active session indicator (green dot) missing for some running sessions

Bug description
When multiple Claude Code sessions are running simultaneously in VS Code, the sidebar only shows the active indicator (green dot) for one of them. The other active session appears as inactive despite Claude Code actively working in it.
Steps to reproduce
- Open two Claude Code sessions in VS Code (Windsurf extension)
- Start work in both sessions so both are actively running
- Check the VS Code sidebar session list
Expected behavior
All sessions where Claude Code is actively working should show the green circle (active indicator).
Actual behavior
Only one session shows the green active indicator. The other running session appears inactive in the sidebar list.
Note: In the Claude Code CLI sidebar, both sessions are listed correctly as active.
Environment
- Extension: Claude Code (Windsurf extension)
- OS: macOS (Darwin 25.3.0)
