[Bug] Plan Mode: Enter key selects wrong default option, requires arrow keys to select "Clear Context and Accept all Edits"

Bug Description
I put Claude Code into PLan Mode a lot. Then it will ask me if the plan is good. I press Enter because it looks like the focus is on "Clear Context and Accept all Edits" but when I press Enter, it says it was rejected for feedback and Claude will ask me what I would like to change. If I use the arrow keys to go down then back up, it will "Clear Context and Accept all Edits" as expected.
