Voice input fails to transcribe: 'Failed to transcribe voice input' error

Summary

Clicking the voice input button in Claude Code's native UI consistently returns \"Failed to transcribe voice input\".

Environment

• Claude Code version: 2.1.72
• Terminal: Warp
• OS: macOS 25.3.0
• Install method: Native install (curl)

Steps to reproduce

1. Open Claude Code in Warp terminal
2. Click the microphone button in the Claude Code native UI toolbar
3. Speak into microphone

Expected behavior

Voice input is transcribed and inserted into the prompt.

Actual behavior

Error toast: \"Failed to transcribe voice input\"

Additional context

• Microphone permission is granted to Warp.app in System Settings → Privacy & Security → Microphone
• System sound input device is working correctly (input level meter shows activity)
• Microphone works in other apps (Teams, Slack, Zoom)
• Issue is consistent, not intermittent
