[Bug] Claude Code ignores instructions and hallucinates implementation details without validating logic gaps

Bug Description
it didn't listen bunch of instractions makes up things and applies that logic, then had to revert back. also it doesn't question the gaps in the logic. we are doing user subscription, but it just implements really bare minimum which misses so many edge cases
