[FEATURE] Add inline feedback option to plan approval prompts

Problem Statement

When Claude presents plan steps for approval, it typically offers 3 options (e.g., Yes/No/Other). When I want to modify something, I have to select "No" first, then explain what needs to change in
a separate step. This creates unnecessary back-and-forth.

Proposed Solution

Always include a 4th option with an inline text input field, allowing users to directly type their modification/feedback and continue in a single step — without needing to reject first and then
explain. This pattern is already used inconsistently in some places and should be standardized across all plan approval prompts.
