[BUG] Claude Code is unable to respond to this request, which appears to violate our Usage Policy

Bug Description

<I keep getting this error on most of my queries, which are just regular prompts with nothing unusual about them: "API Error: Claude Code is unable to respond to this request, which appears to violate
our Usage Policy (https://www.anthropic.com/legal/aup). Please double press esc to
edit your last message or start a new session for Claude Code to assist with a
different task.">

Steps to Reproduce

1. <Start session with: claude --model claude-opus-4-20250514 --dangerously-skip-permissions>
2. <Submit prompt>

Expected Behavior

<I expect Claude to attempt to respond to the prompt, for better or worse>

Actual Behavior

<"API Error: Claude Code is unable to respond to this request, which appears to violate
our Usage Policy (https://www.anthropic.com/legal/aup). Please double press esc to
edit your last message or start a new session for Claude Code to assist with a
different task.">

Additional Context

<This has gone on for multiple (~5) sessions today 7/5/25 ~12:30 PST and after restarting my IDE>
