Plan Mode Rejection Clears Entire Plan Without Feedback Mechanism

Bug Description
Plan mode is semi‑broken. If you say "no" the plan disappears, making hard to reply why the plan was not optimal. Seems have started in <24hrs hours ago and happens everytime.
