[BUG] command 'claude-vscode.editor.openLast' not found on every startup - requires full reinstall to fix

What's Wrong?

OS: Windows 11, VS Code version: 1.109.5, Claude Code extension version: 2.1.51, Claude Code CLI version: 2.1.51. The extension crashes on every startup with "command 'claude-vscode.editor.openLast' not found". The extension appears in Running Extensions but none of its commands register, so clicking the Claude icon or any Claude command fails.

What Should Happen?

aExtension loads previous session gracefully or starts fresh

Steps to Reproduce

  1. Install Claude Code extension and CLI on Windows 11
  2. Use Claude Code normally for a day
  3. Close VS Code
  4. Reopen VS Code next day
  5. Error appears immediately
