[Bug] Session list corrupted after /rename command

Bug Description
After /rename all my sessions are gone. The /resume shows only the renamed session in the list.

• Platform: darwin
• Terminal: ghostty
• Version: 2.1.50

How to reproduce

• Type /resume in Claude Code. A list of sessions are shown. Select one session.
• Type /rename without any title. Claude Code will try to find a good title. Will rename the session.
• Type /resume. Only this session is shown. The other sessions disappears.
• Exit Claude Code. Enter Claude Code. Only this session is shown in /resume.
• However under ~/.claude/projects/<project-name>/ I can find all sessions. 131 in my case.
