[FEATURE] /rename should update the session name shown in /resume list

Problem Statement

When using /rename to name a session, the name only appears inside the active session. The /resume list still shows the auto-generated summary. The /rename name should be used in the resume list too.

Proposed Solution

The /rename name should be persisted and used as the display name in the /resume session list. If no /rename was set, fall back to the current auto-generated summary.
