[BUG] Claude Code CLI responding with unrelated answers

What's Wrong?

Since about 30 mins ago, claude is giving me unrelated responses to my questions and seems to have no context of the project I'm in when using Opus. It is answering questions I didn't ask, and referencing projects I have never heard of before. This is only when using Opus, Sonnet is working fine.

What Should Happen?

Claude should understand the project it's in and have the permissions I have already granted it, and should answer the questions given and not imaginary questions about unrelated projects.

1. Start Claude Code in a VSCode intergated terminal
2. Use the opus model
3. Ask a simple question about the project
