[BUG] VS Code panel sessions inaccessible

What's Wrong?

Title: VS Code panel sessions inaccessible - "Loading sessions" and --teleport errors (v2.1.39 Windows)

VS Code session browser stuck on "Loading sessions..." (both Local and Web tabs)
claude --teleport returns "Error loading Claude Code sessions"
claude --resume works but only shows 2 local sessions; most sessions were created via VS Code panel
Version 2.1.39, Windows 11, Claude Max plan
Tried: reinstalling extension, clearing IDE locks, resetting settings files, killing orphaned processes
