[Bug] Anthropic API Error: Opus model inaccessible from Claude Code despite Team plan access

Bug Description
We have a Team plan where I've been assigned a premium seat and I've not been able to access the Opus 4.5 for 2 days from Claude Code whereas it shows as "Legacy Model" in Claude Web UI.
