[BUG] Usage limit discrepancy: Claude Code blocked at 100% while claude.ai shows 73% on the same account

What's Wrong?

Claude Code /usage and claude.ai Settings > Usage show significantly different usage percentages at the exact same time on the same Max plan account (Opus Plan Mode).
Claude Code /usage:

・Current session: 100% used (completely blocked, all models)
・Current week (all models): 60% used
・Current week (Sonnet only): 11% used

claude.ai Settings > Usage:

・Current session: 73% used (still functional)
・Weekly All models: 42% used
・Weekly Sonnet only: 8% used

Claude Code is completely blocked while I can still chat with Opus 4.6 on claude.ai. This is not an isolated incident — Claude Code consistently gets blocked when claude.ai shows approximately 70% session usage. This contradicts the documentation stating all Claude products share the same usage limit.

What Should Happen?

If usage limits are truly shared across all Claude product surfaces as documented, the usage percentages should be consistent between Claude Code and claude.ai. Claude Code should not be blocked while claude.ai still has significant remaining capacity on the same account.

Error Messages/Logs

Claude Code displays: "You've hit your limit" and blocks all models (Opus and Sonnet), while claude.ai remains fully functional on the same account at the same time.

Steps to Reproduce

1. Subscribe to Max plan, set Claude Code to Opus Plan Mode
2. Use Claude Code normally until usage builds up
3. Run /usage in Claude Code — observe session reaches 100% and all models are blocked
4. Immediately open claude.ai Settings > Usage — observe session shows only ~73% and claude.ai remains fully functional
5. Confirm you can still send messages to Opus 4.6 on claude.ai while Claude Code is blocked

This pattern has reproduced multiple times. Claude Code consistently gets blocked when claude.ai dashboard shows around 70% usage.
