[BUG] /upgrade shows "Upgrade to Max" for existing Max subscribers

Bug Description
The /upgrade slash command description says "Upgrade to Max for higher rate limits and mo..." even though the user is already on a Claude Max plan.
Environment
- Claude Code version: 2.1.34
- Model: Opus 4.6
- Plan: Claude Max
- OS: Windows 11
Steps to Reproduce
- Subscribe to Claude Max plan
- Open Claude Code
- Type
/uto see autocomplete suggestions - Observe
/upgradecommand with description "Upgrade to Max for higher rate limits and mo..."
Expected Behavior
The /upgrade command description should either:
- Adapt to the user's current plan (e.g., suggest extra-usage or show current plan status)
- Not appear at all for Max subscribers
Actual Behavior
The description still says "Upgrade to Max" despite already being on Max.
